Do I need an authorization/referral?

If you are planning on participating with HolistiCare Physical Therapy through your insurance, most companies require an authorization by a physician for your insurance to cover the services rendered.

How do I get an authorization/referral?

You can contact your physician’s office personally, or give us the name of your physician and we can contact their office for you. As soon as we receive an authorization/referral form, we can start Physical Therapy.

What to wear?

We recommend coming to physical therapy in active, comfortable attire, that is appropriate for exercising, and allows accessibility to the body parts being treated.

Where to park?

Parking is conveniently located off of King Street, between Piikoi and Keaumoku. Make a left off of King street to pull into the visitor parking area right in front of the building.

What types of health insurance do you accept?

We accept all major insurances including:

  • HMSA (Quest, PPO, HMO, Akamai)

  • UHC (medicare, quest)
